Job Summary
Join us in making a difference at Home of Guiding Hands as we improve the quality of life for children with special needs. As a Physical Therapist, you will complete evaluations for young children between 0-3 years old who may have developmental delays. HGH offers flexible schedules, reimbursement for CEUs and licensing fees, mileage reimbursements, paid sick and vacation time, medical/dental benefits, and 403b retirement plans, among other perks. Tasks, Duties, and Responsibilities
- Communicates with assessment partner and family to confirm appointment.
- Completes Infant and Toddler Developmental Assessment and intake paperwork.
- Completes M-CHAT review if autism spectrum disorder (ASD) is suspected.
- Reviews SDRC materials with family during assessment.
- Obtains and reviews vision/hearing screening forms.
- Develops report with a descriptive summary of the child’s developmental competencies that include a performance age range.
- Assists in developing relevant portions of Individual Family Service Plan (IFSP).
- Prepares and completes the Provence Profile (IDA) and all intake forms for submission to supervisor within 72 hours of IDA completion.
- Maintains an IDA kit and assessment materials.
- Travels throughout assigned county (San Diego or Imperial) to complete assessments in family homes and other community locations.

Qualifications
Knowledge, Skills, and Education
- A Current Physical Therapy license from the State of California Physical Therapy Board.
- Master’s Degree in Physical Therapy from an approved program by The Commission on Accreditation in Physical Therapy Education (CAPTE).
- Experience providing services for or with children between the ages of four months to 3 years of age with established risk or those at-risk for Intellectual and/or Developmental Disability.
- Must be 18 years of age or older at the time of hire.
- Must receive criminal record clearance from DOJ.
- Must have valid California Driver’s license, safe driving record, and have been licensed to drive at least 2 years.
- Must maintain current and adequate auto insurance.
- Must obtain CPR/FA certification if not already certified (we offer training for new hires).
- Read, write, speak, and understand the English language.
- Proficient in Microsoft Word and Outlook.
- Collaborative-minded professional with strong oral and written communication skills.
- Ability to form positive, supportive, and professional working relationships.
- Detail-oriented with ability to ensure accuracy of program documents and paperwork.
- Ability to manage work time efficiently and meet deadlines without close supervision.
- Ability to maintain cultural awareness and sensitivity in working with differing attitudes and lifestyles.
Physical Requirements/Work Environment
The work environment for this position involves working with a variety of people in their homes and in the community. While performing the duties of the job, the employee is regularly required to see, talk, hear, walk, bend, stoop, sit, stand, reach, and lift objects weighing up to ten pounds repeatedly during a shift.

We support children and adults in San Diego and Imperial Counties with intellectual and developmental disabilities, such as Down syndrome, epilepsy, autism, and cerebral palsy. We provide a full continuum of care including residential care in specialized group homes, independent living support, respite care, and adult foster home services. Additionally, we provide early childhood assessments and services, tailored day support, Life Planning workshops, county-wide transportation services, and on-site activities at our resource center.
